In the box listed school name please list in this order; School/Team, Mascot.
For the second box listed title please write, if you want to add it, Mom, Grandma, Aunt, Sister, Girlfriend, etc.

NOTE: Due to being Women's Cut Name/Numbers will be placed as such. Name down the middle of back. Numbers will be right above name on back see photo for example.
