This is our custom tab, please fill out all information and double check spelling! I will do my best to catch any misspells and reach out! Please select the length of shirt you want and size.

In the box listed school name please list in this order; School/Team, Mascot.

For the box listed colors please list in this order; shirt color, print color.

For the box for Name/Number please double check spelling, you will notice a small charge for this personalization. Unless specified all name and numbers will be placed on back of shirt. Name on the upper back and a number placed in the middle of the back.
